Transition Support Officer
Part time and Temporary position based in Geraldton
  • Part Time - 30 hours per week;
  • Temporary until 30th June 2020 with the possibility of an extension;
  • Based in Geraldton

The Transition Support role will support psychosocial customers to gather evidence and apply for the NDIS. This role will work across all of the National Psychosocial Support Programs (NPS).  

 

Key Responsibilities:

  • Develop the NPS service in the regions
  • Work with customers to gather evidence to apply for the NDIS
  • Develop transition plans with customers to ensure their transition to support services and continuity of services
  • Accept new referrals into the NPS Programs
  • Manage the referrals and budget for NPS Programs
  • Support the team to identify, address and monitor risks, hazards and incidents within individual customer arrangements and report when required
  • Ensure partnerships are established and maintained with external stakeholders such as funders and advisory bodies

 

Selection criteria:

  • Experience in developing programs and strategic program growth
  • Ability to think creatively and practically innovate ways to support people to live as valued citizens
  • Strong problem solving, decision making, planning and organisational skills
  • Excellent communication and interpersonal skills 
  • Excellent leadership and team working skills 
  • Able to confidently and effectively advocate/negotiate on behalf of customers
  • Tertiary qualifications preferred but minimum of Certificate IV or working towards is desirable

In return, you will receive a minimum hourly rate of $29.13 plus 9.5% superannuation, along with the option to salary package with us. We actively support the ongoing training and development of our staff.
