Support Coordinator - Wheatbelt

An exciting new opportunity has arisen within Avivo engaging with our customers to give them choice and control over their supports.  The Support Coordinator will build a persons knowledge, skills and confidence and guide them through the NDIS processes.

 

This is a full time 12 month temporary contract position working from our Northam Office as part of the Support Coordination team.  

 

Our Support Coordinators are responsible for providing support and work individually and autonomously with people with a disability or mental health, their families and natural supports and carers.  Support Coordinators will be expected to provide low level coordination of supports and provide information and advice of the NDIS and provide them with information and referrals and short term intervention.

 

Duties include:

  • Develop a comprehensive understanding of the NDIS and its requirements
  • Strengthen peoples and families ability to connect with formal and mainstream supports
  • Assist customers and support organisations to report on peoples progress
  • Coordinate support from a range of resources
  • Provide advice and support to people and families, in a clear and informed way
  • Resolve service delivery issues
  • Develop participants capacity

 

Selection criteria:

  • An understanding of the NDIS, self direction and flexible support options
  • Ability to connect well with people
  • A well-developed ability to think creatively and develop solutions based on contemporary practice
  • Good attention to detail, problem solving and people-centred decision making skills
  • Well-developed computer skills
  • Excellent communication and interpersonal skills.
