We support people who require individualised assistance to live at home due to disability, frailty or mental illness. We are looking for dynamic applicants living in Armadale, Bentley, Como and surrounding suburbs, with a passion for working with people and a dedication to the needs and wishes of our customers.

 

As a Team Facilitator, you’ll provide quality services to people and form an integral link with our teams. Other duties will include:

  • Providing direct hands on support such as washing/showering, dressing, eating, transport, medication, light household tasks and budgeting
  • Developing a good understanding of the individual and their support team
  • Developing positive relationships with individuals and their families
  • Coaching, mentoring, providing championship and supervising support workers
  • Reviewing and assessing services with the Service Coordinator
  • Organising staff rosters, schedules and providing back up support when required
  • Developing potential resources and relationships within the community
  • Assisting and encourage activities to enhance the physical, social, emotional and intellectual development

 What we require:

  • Previous experience in Disability, Mental Health or Aged Care is desirable
  • Demonstrated experience at a supervisory level
  • A proven ability to work in, lead and motivate a team
  • A current driver’s licence and a reliable vehicle are essential
  • Strong values that align with those of Avivo
  • Demonstrated reliability, flexibility and a positive attitude
  • Excellent communication skills including verbal, written and listening
  • As we provide support over 7 days, good availability to work a variety of shifts

In return, you will have access to some great benefits! A minimum rate of $28.50 per hour is on offer as is a travel allowance of 76 cents per kilometre between supports. Other benefits include the payment of travel time, salary packaging up to $15,900 per annum, and accrued annual and personal leave.
