Avivo work in partnership with people who need individualized support to live at home due to disability; to plan, coordinate and provide supports and services they choose to live their life.
The Team Facilitator plays an important role in our organisation. They provide quality services to supported individuals and they form an integral link between Support Workers and the coordination team. We are currently recruiting multiple Team Facilitators to join our Fremantle team on a part-time basis for 12 month contract.
In addition, this role assists the coordination team in managing effective teams, creating rosters, provides coaching and mentoring to support workers and conducts regular supervision meetings with their team.
Providing direct hands on support
Developing a good understanding of the individual and their support team
Developing positive relationships with individuals and their families
Coaching, mentoring and supervising support workers
Reviewing and assessing services with the Service Coordinator
Organising staff rosters, schedules and providing back up support when required
Developing potential resources and relationships within the community
What we require:
Considerable experience in Disabilities, Aged Care or Mental Health
Demonstrated experience at a supervisory level
A proven ability to work in, lead and motivate a team
A demonstrated flexible, people-centred approach to problem-solving and daily work
Must have availability to work varying shifts throughout the week, Monday to Friday and weekends when required.
A current drivers licence and reliable vehicle are essential
Preferably reside in Fremantle, East Fremantle and O'Connor areas or surrounding suburbs.
Salary is a rate of $28.50 per hour plus 9.5% superannuation and the option of salary packaging to increase your non-taxable income! You will also be paid travel time and kilometres, receive comprehensive training and generate Annual and Personal leave.
